We would like your event at First Assembly to be a successful one. In order to help us serve you better, we ask that you fill out an Event Request Form. Upon completion of this form either turn it in to office by fax or email.
Forms will be reviewed for approval every Tuesday Morning. Please turn in your Event Request form at least four weeks in advance of your event.
